A quick way to tell whether the fog is outside or inside while driving: if the wipers clear it, the fog is on the outside. If the wipers do nothing, it is on the inside.
Common Causes of Fog Inside Your Car

Your breath. Each person in the car adds moisture to the air with every breath. Four passengers will fog up a car much faster than one.
